However, their peaceful life is interrupted by the appearance of Sakuta’s first love, Shoko Makinohara. With this, Sakuta meets two Shokos, one in high school and the other an adult, the first to help her with her memories.
Sakuta discovers that Shoko High School is suffering from heart disease and urgently requires a heart transplant to save her life. When both Sakuta and Mai worry about Shoko’s serious illness from high school and think of ways to help her, Sakuta finally discovers the link between her injuries and the adult Shoko: the adult Shoko comes from the future after a successful surgery from the transplant of heart.
Shoko’s heart donor is Sakuta himself, whom Shoko says was declared brain dead after a fatal car crash on Christmas Eve. It turns out that the reason Shoko returned was to save the life of his beloved Sakuta. Against Mai’s will, Sakuta sacrifices herself so that Shoko can live. However, the couple promises to meet and fall in love again. Sakuta visits young Shoko for the last time, where he reveals that he now knows everything that happened. He decides that he will create a future in which he will never meet Sakuta and Mai, thus avoiding them from the sadness of the state.
Time goes back to the past when Shoko was in fourth grade. He courageously writes down his future hopes, but that means letting go of the memory of having met Sakuta and Mai; and that they would probably never meet her. Finally, time returns to the present. Thanks to pieces of memory that cross different timelines, Sakuta and Shoko finally recognize each other.
